NEW PAYMENT POLICY
ATTENTION: ALL PATIENTS
The office will be instituting a New Payment Policy on December 1st 2024.
We will require a credit card be kept on file for payments on your account. Following are
documents that you can download. Attached is the Announcement and Frequently Asked Questions FAQs form for additional information and the Credit Card on File Agreement document. The agreement document will be need to be signed, completed and submitted to us before or during your next appointment after December 1st 2024.
